During your first visit Dr. Brokaw and his team will perform a Comprehensive Oral Examination. This will include examining your teeth and gums, performing an overall oral health and soft tissue evaluation, a review of the appropriate radiographs (x-rays), and make an assessment of your present oral health condition.
If any dental treatment needs are discovered, a treatment plan and financial estimate will be prepared for you prior to beginning any dental procedures. Once a plan has been agreed upon, appointments can be scheduled that best suit your needs.
Dr. Brokaw and his dental team pride themselves on being timely for each and every visit, and we ask our patients to provide 48 hours’ notice if their appointment must be rescheduled.
For extensive treatment plans, a complementary consultation will be scheduled once Dr. Brokaw has prepared treatment plan and financial options and to give you the opportunity to review recommended treatment and ask questions.
Please complete all of the patient registration forms and bring them with you to your first appointment. If you have dental insurance please be prepared to provide us with your insurance card and any additional insurance or payment option information as well. We are happy to obtain your insurance benefits and will assist you in filing your claims.
Payment Options and Insurance Filing
Matthew R. Brokaw, DDS, PC requires that all patients render their estimated co-payment for service provided at the time of each visit.
We are committed to doing all we can to make your treatment affordable and stress-free. As a courtesy, the business office will be happy to file your insurance claim.
Please keep the office up-to-date on any changes to your insurance to insure timely processing of your claim.
We accept Cash,Check, & All Major Credit Cards. Payment plans are also available through CareCredit. (Please ask our Financial Coordinator for details.)
